Effective
April 30, 2020, PharmaCare Plan W has added Sublocade™ (buprenorphine) to the
list of Limited Coverage Drugs available to Health Benefits clients. Sublocade™
is a subcutaneous-injection, long-acting formulation of buprenorphine.
Limited
Coverage Drugs, including Sublocade™, require Special Authority for coverage. Your doctor can
submit a Special Authority request directly to PharmaCare.
